I would look on your local council website or see if it tells you on your badge anywhere who to contact

As @janer1967 has already mentioned, you'll need to get in touch with your local council to change the address for your Blue Badge. 

Here's a link to the 'Change the details on your Blue Badge' Gov.UK website which allows you to input your new postcode and find out the relevant council contact details. Just to make you aware that there is a separate system if you're based in Northern Ireland, but the website has some further details on that for you as well.
