Any advice when filling out form for PIP?

My daughter has ADHD and anxiety and it’s been suggested she apply for PIP.
It’s probably me that will fill the form out so do I complete it as her or as the second person?
Any other advice most welcome as we are both so new to this and her diagnosis.
