Claiming PIP - but what about disability premiums?

I am reading on the GOV website that if you qualify for daily living PIP you are also entitled to Severe Disability Premium (£67.30 per week).
How do I go about applying for this? It says on the website that it will get added to your existing benefits, but which benefits exactly?
I would be receiving PIP every 4 weeks, UC every month, and I am currently also awaiting a decision on my ESA assessment - in a month or two I might not even be claiming UC, I might be put back on ESA!

I'm assuming here, but do you mean New Style ESA?0

You cannot get premiums in UC or new style ESA.
They only exist in the old income based ESA.
If your new style ESA is reinstated you may receive both ESA and UC but the ESA will reduce the amount of UC you receive,0
