Hello,
I’m currently in receipt of ESA and in the support group.
On the 25/03/2020 I filled in a ESA50 form which I received from the DWP and posted it back.
In September 2021 the Health Assessment Advisory Service asked me to attend a assessment in York which was a 140 mile round trip.
i declined due to my disabilities and chronic cluster headaches, a telephone assessment was agreed.
Unfortunately they made a hash of things and continued to send letters out asking me to attend a face to face in York, this went on until I made an official complaint and now have a telephone assessment mid February 2022.
My only concern is that the information they hold on the ESA50 form is now nearly 2 years old and out of date.
I have sent the Health Assessment Advisory up to date consultation letters showing a clear deterioration in my disabilities but this still concerns me massively due to the lies and untruths told on my first ESA assessment.
This resulted in a complaint to the DWP from Citizens Advice then another assessment was completed properly this time with an advocate by my side.

The ESA50 form is available to download and print online, https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/975770/esa50-capability-for-work-questionnaire.pdf
you can do this and send them before your assessment. Just ring them to make sure you know the address to send it to.0 -

Have done this and will update the information and email it over to the Health Assessment Advisory Service hopefully in time for my assessment on the 17/02/22.
