The review seems very basic...but knowing Pip the details in the nuance.Its given me only 2 1/2 weeks to fill it in and get it to them...Called to have it extended.As my illness co morbidities are always on the gradual move towards further debilitation. When they ask "when did it get worse" its hard for me to give a definitive time.. I dont want to go crazy with over thinking this form, but equally dont want to under sell the very real situations i am in daily... I can go from 80% to 100+ in a day on pain and functionality.. Recently ive had a 4 week constant 100% to the point of sheer exhaustion so have been prescribed liquid morphine as a break through drug to give me at least a rest from the constant barrage....Is there anything i need to particularly know about this form? any heads up would be appreciated.

You need to treat it as a new claim. Include a couple of real world examples of what happened the last time you attempted that activity for each descriptor that applies to you. With details such as where you were, what exactly happened, did anyone see it and what the consequences were.Use extra sheets of paper because there's never enough of room on the form. You should aim for at least half of an A4 sheet of paper per descriptor that applies.0

Renewal/review are both the same. Regardless of that it needs to be treated as a claim.
