I’m feeling very anxious today as I have to fill in my pip application, I applied 3 years ago and got refused. I really don’t want to do this because being refused makes me feel worthless and they made me feel as though this is all in my head

I know it's hard but try not to be anxious and your certainly not worthless.

When filling out the forms you should put a couple of real world incidents per activity that applies to you.
Include information such as where you were, what exactly happened, did anyone see it and what the consequences were. You should aim for at least half an A4 side per descriptor that applies to you.If you need extra time to return the form you can ring DWP to ask.Expect an assessment eventually because most people have them. Timescales from returning your form to having a decision is about 10 months.When you previously applied and were refused did you challenge that decision? If not, was there any reasons why? If you’re refused again then you should get some expert advice to check that you can score enough points needed for an award.1
