Who do i request a copy of my health assessment notes? It was carried out by Atos...
As mentioned numerous times before on my posts...I received a random call...(no notification that it was an official call and the last chance i had to give evidence and further clarification.)
So im extremely concerned over what the assessor has written. Given the way, she only asked me 2-3 questions ,and didnt inform me that it was an interview .
Plus the fact she knew one of my conditions ,puts me completely at a loss,by being approached like this (at least she would have seen the evidence there as i had no clue how to reply coherently at the time, and had just woken up from the sofa.
She didnt even ask how i was!
So with zero notification of an interview date(forms sent in Feb 22)no letter nor text ,Nothing!... This conduct regrettably leads me to believe, that i cannot trust the report.
So who do i ask for a copy of said report?
If anyone knows, Many thanks in advance.

You need to ring PIP 0800 121 4433. As mentioned in your other thread it would have been a paper based assessment you had and they just needed to ask you a few questions. They don't need to give you any warning of this because it's not an assessment.Speaking from personal experience with paper based assessments i can only give positive feedback. Just remember the assessment report is not the decision, it's a recommendation.1

I'm not surprised @Middleton. PIP sounds really confusing! It is more of a reflection of PIP than it is a reflection of you. Please don't hesitate to let us know if we can do anything else to support you or if you have any other questions0
