There have been many times in the past when keeping track of all my contact with my different doctors for my different mental and physical disabilities would have saved my ass, so I put together a printable tracking log and a spreadsheet you can customize to your particular needs and medical conditions. I live in the US, but I have a friend struggling with the NHS to get good mental health treatment and she seemed to find it really helpful, so I'm posting it here JIC somebody here may find it helpful too!
