In a previous comment you said you’ve been sending fit notes since Feb, were those accepted?
May I also ask were you sending fit notes at anytime during 2018/19?

All my notes were accepted except the last two ( which were the same ) and from 15 sep until now have all been for the same illness.

That is 15 sep 2021 until present all sick notes had been accepted . Sorry if I'm making this hard work for you.

Seems very odd, if you were found fit for work they don't usually accept any further fit notes, unless it's for a worsening of an existing condition or for a new condition.
Can i please check that the letter you mentioned last night was definitely a work capability assessment decision letter? If it was then it would say on the letter.

I received  a letter via my journal 23 Aug 2018 stating they were sending me a form to fill out, I never received  any letter or form. On 10 Jan 2019 I received a letter via journal saying I did not attend said appointment  but I had no form or letter informing me of such appointment  therefore I didn't know I'd even had an appointment to go to.

That's odd because usually when you receive the letter on your journal telling you they are referring you, then usually you receive the form shortly after.
Are you sure you didn't receive it and forget?

No I've never received this form I don't even know what it looks like apart from people telling me it's like a little booklet, but I'm not worried about that , that was in 2019 its my current illness that I want the referral for as you see from my dates  the 2018/2019 I only had two sick notes and I was capable of work I'm just peeved that they are using 2019 to decide this illness , as they have been accepting my sick notes for this illness since sep 2021 which is  what I want to be referred for and the 2018/9 was a different illness

And if they have been accepting them they are agreeing I'm sick  so how can I be fit for work and sick at the same time I just can't seem to make sense of it all according to them .

That's not what you said in a previous comment here.

You can be found fit for work without filling in a form. It's impossible to advise without all the information because i can only advise based on what you tell me.
I'd advise you to speak to an advice agency near you and then you will be able to log into your journal and show them everything and they will be able to advise based on the information in your journal.
Start here. This link will tell you what agency is local to you. https://advicelocal.uk/welfare-benefits
