Does anyone have a template I can use for my ill health retirement statement?

Hi does anybody have an example or template I could use please?
I am applying for retirement on the grounds of ill health and could do with some help with my personal statement (part B)
Work for the NHS.

Not a template as such but this is along the lines of what you might want to include. Being honest is important as the medical Officer will know from the supplied medical evidence the described and accepted symptoms of your condition.
