Hi and welcome
Is this for a review or a change from DLA ?
I have heard that the DWP do have a service to help with form filling. You might try the PIP help line

Change from DLA for PTSD to PIP to include my brain injury from June 2016.

Only thing I can find is the Royal voluntary service might be worth trying
