Help with ESA form

Hello i put in a claim for my son im his appointee for ESA backating changing from incapacity benefit too ESA they sent me a ESA3 form too fill out again with correct national insurance number but differen claimant name im furious asking for information which dosnt even relate too my son they have suspended his claim now any answers for please frustrated

Thankyou im very upset with this as form dosnt say my sons name he never had any.one move in with him but form asking for wage slips for her very upsetting

Scope -
You might want to point out that "they" have broken the Data Protection Act by giving you the name of a claimant with whom you have no relationship so would be grateful if they could provide correct forms asking for correct information without you having to report them to the Commissioner, " as you do not wish to cause trouble."
Bearing in mind that it is a massive fine for what they have done they may become a bit more helpful. . -

Have you considered contacting your local Citizens Advice for help with the ESA form? You can also find more information about Employment Support Allowance and the forms on their website, which might be helpful as well.
