Challenging a PIP decision

Good morning
I recently received my decision of my PIP Application which resulted in the answer been I don't qualify for pip payments. I feel that DWP and the assessor have not taken any notice of my evidence or what I said in my assessment.
Can anyone give me any advice about filling out the form to challenge their decision. I'm not sure what other evidence I can send them as I've sent everything to prove my illness and condition. I'm a sufferer of anklosing spondylitis, arthritis and recently had spinal surgery to remove a disc and decompressions.
