hey all new here
I’ve lost all my medical letters and old esa forms should I be worried .? I have not had new esa form yet so am I worrying to much ? Or should I contact go and jobcentre for copy’s ? Need some advise thank you

Aww ty I use them for my esa form when it comes but I,be lost all my papers from way back 2010

You could request a copy of all your medical files from your gp in the form of a subject access request. Ask them if they have a form to fill in. Takes 30 days and it’s free

Just had to order new birth certificate I lost that too in my folder not my night ?

You can ask for copies of all your medical notes from your GP.
However, when it comes to filling in a capability for work questionnaire, it's your condition now and how it affects you now that will be taken into account when the DWP make their decision, so although you can ask for them if you want to, you shouldn't worry too much if you don't have them, and if you have any recent medical letters then they will be more useful than any older ones.

Ahhh ty so much lee that so true this year I’ve had 2 eye surgery and my eyes got worse and over things. Guess I wait for new form then
