Advice on Tier 2 Tribunal

Just come across this forum whilst looking for info on applying for a tier 2 tribunal for PIP. My award was reduced to standard so I went to tribunal but was refused so have been advised to request a tier 2 tribunal after receiving statement of reason ....is there anyone that can advise or if there is a template letter that can be accessed to apply to upper tribunal . Many thanks
