The onus is on you ti provide evidence not them
What was you going to send as it needs to be relevant anyway0 -

I have written everything in the form and along with that I sent a letter from support worker but I couldn't send any physical health evidence now I'm thinking why I did not sent it this feeling of not sending making me soooooo stressful0

Try not to worry physical medical evidence is only relevant if it details how your condition would affect you in a work capacity
If they don't detail this then they are not required
All I sent was my medication list and my certificate of sight impairment
The main thing is a well detailed form0 -
Hi I found the address where I can send my evidence to the universal credit assessment team it was on the letter which they have sent me with the questionnaire form0
