PIP appointee. Do they sent the application form, or do I have to ask for one?

Just received PIP appointee confirmation for my child who is 16 in June (form B57) . No interview or phone call. 
What happens next? Do they send the PIP application form or do I have to ask for one ? 
Why would they grant appointee status if we haven't even applied for PIP yet ?

    Usually you need to have the appointee in place before any forms can be sent. This is because the forms can't be sent to your child, they will be sent to you, in your name on behalf of your child.
    If you haven't applied for PIP yet then you will need to do that next.

    By granting me appointee they are acknowledging his disability. That is something I can use for evidence ?

    Not directly although the reasons he needs an appointee are likely to be relevant.
    PIP is awarded (or not) solely based on the difficulties experienced carrying out the prescribed activities to the required standard.

    No, it's not evidence that will support a PIP claim. It has been known for a claim to be refused even if they have an appointee and i'm not saying this will happen to you. Just pointing out that it's not evidence.
