Hello @n31 I hope your Thursday has been a good one.
Would you please be able to explain a little more about what the proof is needed for? That will help us better support you.
Many thanks

To get disabled persons bus pass down my part off the country I had to get a social worker to sign a form saying child had x y z . Don’t even have diagnoses so couldn’t advise if you could even use that form .n31 said:For a bus pass and uni0 -
Could a GP do it?0

@n31 Are you wishing to access your University's disability services? If so, I believe the diagnostic report will be enough if completed by a professional. Each university has different processes though so I'd contact your institution directly to double check.
Will you be applying for a disabled person's travel pass with your local authority?0 -
OK and yes0

I just realised that the diagnostics document has the wrong dob can the G.P change this or must it be the Dr that diagnosed?0

For a bus pass, if you mean a disabled bus pass then your ASD report may not be enough of evidence that you qualify. You'll need to look at the eligibility criteria in your area.In my area it's not enough because it wouldn't qualify you. For my daughter she needed a diagnosis of a learning disability. Or a Social Worker to sign a form saying she has a learning disability. For Uni then it will depend what it's for exactly. I don't think a GP could change the D.O.B on your ASD assessment report. For this you'll need to contact who ever it was the diagnosed you.0

Hey @n31. In terms of proof of autism for uni, I needed the same thing when I first applied for DSA!
I needed to ask my GP to fill out a form given to me by DSA. This form required my GP to include things like ticking it would last 6+ months and the severity.
Wishing you the best of luck with this. Please don't hesitate to let us know if there's anything else we can do to support you
