Hello @n31 I hope your Thursday has been a good one.
Would you please be able to explain a little more about what the proof is needed for? That will help us better support you.

Could a GP do it?

@n31 Are you wishing to access your University's disability services? If so, I believe the diagnostic report will be enough if completed by a professional. Each university has different processes though so I'd contact your institution directly to double check.
Will you be applying for a disabled person's travel pass with your local authority?Online Community Co-ordinator

I just realised that the diagnostics document has the wrong dob can the G.P change this or must it be the Dr that diagnosed?

For a bus pass, if you mean a disabled bus pass then your ASD report may not be enough of evidence that you qualify. You'll need to look at the eligibility criteria in your area.In my area it's not enough because it wouldn't qualify you. For my daughter she needed a diagnosis of a learning disability. Or a Social Worker to sign a form saying she has a learning disability. For Uni then it will depend what it's for exactly. I don't think a GP could change the D.O.B on your ASD assessment report. For this you'll need to contact who ever it was the diagnosed you.I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help.

Hey @n31. In terms of proof of autism for uni, I needed the same thing when I first applied for DSA!
I needed to ask my GP to fill out a form given to me by DSA. This form required my GP to include things like ticking it would last 6+ months and the severity.
Wishing you the best of luck with this. Please don't hesitate to let us know if there's anything else we can do to support youCommunity Volunteer Adviser with professional knowledge of education, special educational needs and disabilities and EHCP's. Pronouns: She/her.
