How to add a new medical condition to the UC50 form?

In the beginning of January 2024 I filled an UC50 form and I sent it. About 10 days ago I had a phone health assessment. I would like to know how can I add a new medical condition to the UC50 form because when I sent it in the beginning of January 2024 I already had this medical condition but it had not yet been diagnosed because I had not yet an appointment with my doctor about it and as a consequence I did not make reference to it in the UC50 form

    You won’t be able to include it with the form now because you’ve already had the assessment. The assessment report has most likely been written and returned to UC. 

    You could send it to UC but please note that it’s not about a diagnosis. 

    Please also be aware of reporting new conditions on your journal because this could prompt them to refer you for another work capability assessment. 

    You could also leave it and if the worst happens and you need to challenge the decision you can then send that as extra evidence.

    I would like to know if it is possible to have another health assessment which will be a small one concerning only my additional medical condition

    There's no such thing as a "small" assessment. If you haven't yet had the decision of your most recent assessment then you will need to wait for that. It's never as easy as just having another assessment. 

    As I advised, it's not about a diagnosis.
