Recived information after migration letter

Hi were in the middle of migration to UC. I work and also care for my wife we used to just get Working Tax Credit and and PIP carers allowance and full mobility no other benefit. Now that we had to change I received this letter does this mean carers allowance stopping ? it all seems so hard and she's getting hounded with emails telephone calls also now has a phone call weekly to get her back into work she is suffering terrible and this is making things 10 times worse. I feel awful any advice,

Carers allowance continues as normal but will be deducted in full from any UC entitlement. You need to make sure you report being a carer and they will include the carers element in your maximum entitlement.
Your wife will need to make sure she reports her health condition and provide a fit note within 7 days. She will then be referred for a work capability assessment.When claiming UC she will be treated as a job seeker until a decision on a work capability assessment says otherwise. It’s a work coaches discretion to either turn off or reduce commitments.0 -

Does it mean i have to go somewhere as I'm telephone appointments. Thanks

It's a work capability assessment. You will receive the UC50 form in the post in a few days. You will need to fill that in and return it no later than the date in the letter, with all your supporting evidence.
An assessment is most likely because most people have them. They are mostly by telephone but some are face to face at an assessment centre.0
