Learning Disability Diagnosis

Hello, I am a Mum with a 21 yr old who is Autistic and also has a Learning Disability.
She has a friend from daycare who I am sure has a learning disability, but she is not diagnosed.
I am sure I saw a template form that could be completed to send to the GP to request that her learning disability could be confirmed, and she could be added to the LD register and get free health checks. The family have English as a second language so I am asking on behalf of the friends Mum. Any advice welcome (the friend is 20)

Hi @Criss01 welcome to the community!
I'm not too sure about the form you've mentioned, perhaps one of our other members might know more about that. But if your daughter's friend or caregivers speak to her GP they can take her through the steps to being put on their learning disability register. There's some information and advice about it on the Mencap website if you'd like to read it or pass it on.
If her disability isn't yet diagnosed, the GP should also be able to advise what the next steps might be to make sure she's getting all the support she needs. Most GP surgeries will have services for patients who need to talk in their native language, so there should hopefully be some kind of help available if her mum needs a translator 😊
