Access to journal notes
does anyone know if the health professionals who take LCW assessments have and read all the notes made by your coach before they decide weather to do assessment or make paper decision.

No, they do not have access to your journal.

The health professionals who do the WCA assessments are not employees of the DWP, so as Poppy says they will not have access to the journal.
You should make sure that if there is anything in your journal notes that would be relevant to an assessment you should make sure to mention it during the assessment.
Let us know if you do have anymore questions, and if you would like you can read more about the Work Capability Assessment on our advice pages: Work Capability Assessment | Disability charity Scope UK

sorry to bother you again but during the assessment for my partner, if I am the appointee will she still have to sit in front of the screen if it’s not face-to-face or can I represent her?

As her appointee you will be expected to answer the questions they ask.
