PIP 1021 form help

Hi,
Has anyone had to fill in a PIP 1021 form for award review.
Just wondering what to write in the descriptors which have not changed as the form states write if things have stayed the same.

You need to treat it as a new claim and give as much information as possible, even if there's been no changes. Include a couple of real world incidents of exactly what happened the last time you attempted that activity for each descriptor that applies.Add detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.You should aim for at least half an A4 side of paper per descriptor that applies to you. Make sure you add your name and NI number to everything you send and keep a copy for yourself.0

Have you received a PIP1021 form, which you receive after reporting a change of circumstances, or a PIP AR1 award review form? I'm just puzzled as it seems you had a successful Mandatory Reconsideration in January, & thought you were pleased with this award which was until 2025.

Hi, Sorry.
I have received the PIP 1021 due to my mobility it's deteriorated drastically since the award and I requested a worsening condition, the form is very short and asks if there are any changes but my mobility has only changed.
At the time I was pleased but cannot walk without any pain and discomfort.0 -
I hope you got some expert advice before reporting the changes. Please see my previous comment about filling out the form.0

hi. So towards end of 2022 I sent off a new claim for pip and was officially then awarded start of 2023 until 2027.
Last week 2nd November 2024 I received a letter in the post from pip sep “making sure your award is right” at the top and also says “thanks for telling us your needs have changed” it’s. Pip 1021 form.
I understand random reviews can happen at anytime but have they sent me the wrong form out as I haven’t informed anyone my needs had changed as I was happy with the outcome on my award.
I now have 2 weeks apparently to return the full form or I could lose my pip according to the letter.
Can anyone please advise.0 -
