    I would like to know if I can make a Right of Access Request to the Information Commissioner Office (ICO) to know the name of the Healthcare Professional who access me during my health interview concerning my ESA claim.

    I think that I have the right to know the name of my JP who is a third party so why not also the name of this healthcare professional who is also a third party?

    Peter, have you appealed the decision(s)?

    I received unredacted documents related to my claim and the health assessment from the Tribunal before the hearing.

    My Right of Access Request (RAR) files from the DWP have almost all the names redacted.

    Every IB85A every ESA85A every UC185A in my file is a Medical Report Form.

    There exists an ESA85 an ESA85a and an ESA85A - one is for a paper-based assessment, not sure which of the first two that would be though.
