You need to apply via your local authority.
There are automatic ways to qualify, as well as other criteria you might qualify under.

That's not exactly correct because for PIP it will depend on where exactly you scored those points. If you scored at least 8 points in the moving around part of mobility then you'll automatically qualify.
For those that scored for following and planning a journey then you need 10 points for activity 1E, unable to undertake any journey. If you scored points anywhere else, even 12 points for this, you're not automatically entitled to the BB.
