You're welcome. I agree it's badly worded but I would take that to mean additional evidence after you return the form.
I would say there's a slim chance you will receive it back within that time. Reviews have extremely long delays so I would finish off filling it in and send it with any additional evidence you have. Then once you receive the report send that as extra but include a brief letter with it.
Please don't worry about asking questions, there's always someone here that would be happy to answer them for you.

In my experience it can take up to two weeks to receive post from the DWP. Have you already asked for an extention on returning your PIP form?

My form is due back on the 19th January.
I have now requested the assessors report after an amazingly long wait on the phone which should take 10 working days. I need to check a couple of things in there before returning my form. It's unlikely it will get to me before I need to return the form anyway.If I return my form on the 16th or 17th it obviously won't get to them by the 19th so what do I do there? Will it stop or what happens?

I would do as I advised yesterday. Return the form as soon as possible and then send the assessment report to them at a later date.

I will do but even with the best will in the world I won't get this form done until middle of next week. I am struggling with a lot of things at the moment and even writing a few words is taking hours.
Which takes me back to what I said, if I return my form on the 16th or 17th it obviously won't get to them by the 19th so what do I do there? Can I ring them and say its been posted and hope they don't stop my pip?

If you send it on the 16th, there is a chance it could get scanned onto the system on the 17th, but I would say the 17th would be too late to send it.
They may temporarily pause your PIP but if you call them and let them know when you posted it, they should reinstate until your review.
Also, I know you already had a 2 week extension, but they can extend the date for a maximum of 4 weeks. Have you considered asking for a further extension to give you time to receive the assessment report and give you a little more time to fill out the form? If you explain to them what you've said above, that you're struggling with lots of things at the moment, I can't see why it would be an issue for them to extend.

Thank you for the comprehensive reply. No I didn't realise I could ask for a further extension. It took me an hour to get through this morning and didn't even think of asking :/
I will give them a call tomorrow and hopefully they can extend it. It will make things a lot lot easier that's for sure.

Hi Poppy, your advice here is amazing and I have learnt lots from reading your advice. Do you mean that you can send in your previous assessment report as evidence for your next review? I've never seen a copy of any of my reports, do they ask why you want it or do you just ring pip and ask them to send it to you. On my last my head was a mess and I did just write no change on most of the form and obviously had to be assessed again so reading through all your advice has been great and may even take some of the anxiety out of my next review so thank you. @Ray212 My review took almost a year, get those forms sent off then try to put it to the back of your mind for a while.

Many thanks. I am going to call them tomorrow to see if I can get it extended further, if some reason I can't then I will do what I can and send that.

Ok, gave them a call first thing and they have extended it to the 2nd February so gives me a lot more time now to get this done, get the report and get the form sent off the first time so I don't have to worry about it further.

Thank you, that's very kind of you. You can send the assessment report with your review form but they do sometimes refer back to that during a review because they keep their copy.
You can ring PIP to request your copy and they do not ask any questions why you want it.
I'm glad my advice has helped you. I don't usually advise people to just write no change, all claims should be treated as a new claim and you should give as much relevant information as possible.

Well wonders will never cease, the report showed up this morning. Only asked for it Monday.
Anyway wanted to ask something, at the end of the form it says :
"It is likely that the functional restriction identified in this report will be present at the recommended point of review" and the box is ticked yes.
What does that mean exactly?

So I am still doing these review forms, literally just got around to it today, kept putting it off.
With hospital letters etc is it ok to send copies or do they have to be originals?

They ask for copies, which is best as nothing will be returned.
