    Need some help making sense of the assessors report. There are shortened words and I don't know what they are, they are referencing :

    FH

    IO

    MSE

    HOC

    Any ideas?

    It tells you on the report somewhere what the abbreviations mean.

    Where would you mention about cpap machines and causing sleep disruption, fatigue etc?

    Is there a box that asks for any other information? If so then put it in there. Otherwise just include it on additional sheets of paper.

    Will they pay attention to it as its not in the "main" sections? I believe Section 6 has a "further information" bit.

    They will look at the whole form, including any additional information you send as well as any medical evidence.

    Was this question always like this, the medication question. I can supply a list of medication but it asks :

    What your medication is
    Dosage
    Frequency
    How it affects you ←—————— Was that always there?

    I don't really like this longer form, seems to make you really need to work at it and bog you down with multiple answers.

    I am getting a bit bogged down with this form. And I have to have it sent off by Tuesday.

    Is it worth getting the benefitsandwork guide which has apparently been updated?

    As there are new parts added to questions, for instance, medication, it now asks are you taking these as prescribed, with just a yes or no response where for me there is an inbetween.

    Wondering if its worth getting the guide but it costs money.

    Hi @Ray212 have you thought about contacting Citizen's advice as they should be able to help you fill out the forms in one go and answer any questions you have. Just thinking with you mentioning you're getting bogged down with it, it might be beneficial to try that rather than spending money if it's quite costly. I've added a link below for their website where you can find your local branch.

    I don't have time now. I already have had two exensions and its due in on the 2nd February so need to post it off beginning of the week to make sure it gets there in time. Last thing I need right now is them stopping payments because they haven't received the form

    This form is so bizarre. So Section 4, question 21 asks about any treatment/therapy etc you have had since your last award.

    All you can answer is 1)name of treatment, 2)when did you have it or 3) how often did or will you have it.

    It then says you can tell us more about how you manage your treatments in Section 5, Activity 3. But that question is, Can you manage this activity safely and without difficulty. You can only answer yes or no, if you answer yes you have to go to the next question.

    Which is bizarre as I can manage safelt and without difficulty but doesn't mean the treatment itself or the device I use isn't painful etc. There is no where you can enter how you manage your treatments UNLESS its unsafe and difficult

    You can include extra information on separate sheets of paper, just make sure you state clearly which question is refers to. You don't have to follow each question exactly as it is in the form.

    This new style form seems to be laid out in a way where it stops you from entering vital information. As in my example above, tells you to say how something affects you but doesn't allow you to say anything unless its unsafe or with difficulty.

    Thanks Poppy so thats good to know. I have spent days and not got very far at all. Mainly because of the silly wording of these questions. I will just fill it out as best I can.

    Silly question maybe but I have almost finished this. Most of the questions I have answered I have said "See attached sheet" but the sheet isn't attached, its just loose with the supporting documents. Will that matter?

    It doesn’t matter. Even if you use staples, they would be removed once everything is added to the system anyway. Just make sure you include your name and NI number on everything.
