Cerebral Palsy and transition support.

Hello, my name is Heather and I am an occupational therapy apprentice studying at the University of Brighton. I am currently on a diverse practice placement with Sussex Community Foundation Trust and am researching young people's lived experience of therapy transition support.
If you are 15 or upwards in age, have CP and are in mainstream education I would be interested in hearing how you are being supported to make the transition from children to adult services. I also want to know where the gaps in service are for young people living with CP, particularly if you live in Sussex.
