Mistake on DLA form

Looking for a bit of advice.

I sent my daughters DLA renewal forms back today.

I did a full day in a diary form including night time of a day whilst she was on her period to show how much extra support she needs at these times.

I included an extra night time page for a day she wasn’t on her period to show her care needs on that day too.

Looking through my copy’s now I realised I’ve labelled them wrong , I added the non period day in the full day as ‘page 4’.  On the actual period day I labelled it this is a non period day!

I was rushing and adding labels on the last minute.

I don’t know what to do. The DM is not only going to be confused but it looks like I’m giving false information because I’ve obviously wrote she changes period knickers etc on non period days?

Will they realise my mistake or shall I contact them/write to them? 

I’m worried I’ve messed my daughters claim up and not sure what to do.

    @loz21x Give them a call and explain, to save you a lot of worry. Any one can make a mistake, don't worry about it.

    It's okay to call or write to them to explain about the situation. It shouldn't harm the claim and it shows you're being honest, hope that puts your mind at ease😊

    Thank you for your replies.

    Im so annoyed with myself I spent 2 weeks doing this as I get so overwhelmed.
    Made sure I had completely factual information by doing diary’s etc.

    Then in the last second before posting I mess it up doing that.

    I was extremely worried because it’ll look like I’m saying she wears period knickers n gets upset over them on days she doesn’t even have periods.


    As I only sent it yesterday I think I’ll write to them today and explain my mistake, hopefully it will make sense to them and they won’t take my daughters award away because of this or look like I’m giving false information.


    Her needs have increased dramatically since last award so I was trying so hard to paint a full picture of mobility/emotional/lifestyle struggles.


    if I send a letter will that just be added to my renewal forms?

    Please don't be hard on yourself, these things happen and DLA forms are very stressful in general @loz21x

    If you send it to the same address you sent the original form to and include the claimant's name, address and DLA reference number you should be okay and it should all be looked at as one and not have an impact 😊

    Thank you.

    I couldn’t sleep last night with worry.

    Although it’s a minor mistake on the title it could hugely impact what I was trying to put across and different levels of care needs.

    Thank you for your replies and help and I will send a letter out today - tracked.
