PIP are saying that I missed my assessment on the 18th June they phoned me I answered the phone every time they phoned but kept losing signal I just managed to say hello then it went if they check there records for that day and the next day cos every time they phoned I only managed to say hello before I lost signal now I have a form to fill in asking why I missed the assessment with evidence but I don't have any thing to back it up so I don't think if it is worth filling the form and sending it back

Hi,
You don't need any evidence, just explain what happened, very poor phone signal.
If you have a landline, perhaps give them that number as an alternative to use.
