Hi @JAH and a warm welcome to the community from me!
It's understandable why you're feeling anxious, moving awards can feel quite overwhelming and uncertain. Here is some information from Motability about moving from DLA to PIP that I hope will be helpful. How are you feeling about filling in the PIP form? -
Hi Mary_Scope thank you for the information that is very helpful. I’m nervous about filling in the form , having listened to so many horrendous outcomes.

How have they let you stay on DLA for so long, I can understand why this is going to be so scary, I hope it goes well for you.
My advise is to read up on the descriptors, make sure you understand them, when you fill in the form, explain how you meet the descriptors, that is more important than describing your medical conditions.
Any supporting evidence of course supply with the form, but I personally its more beneficial to just supply effective evidence supporting the descriptors, rather than sending a big bundle of your entire medical history.
If this simply feels too uncomfortable, see if you can find a local advocate to help you with the process.
