    Need some help making sense of the assessors report. There are shortened words and I don't know what they are, they are referencing :

    IO

    MSE

    HOC

    Any ideas?

    It tells you on the report somewhere what the abbreviations mean.

    Where would you mention about cpap machines and causing sleep disruption, fatigue etc?

    Is there a box that asks for any other information? If so then put it in there. Otherwise just include it on additional sheets of paper.

    Will they pay attention to it as its not in the "main" sections? I believe Section 6 has a "further information" bit.

    They will look at the whole form, including any additional information you send as well as any medical evidence.
