Tell them about your conditions and how they affect you but you should avoid telling your life story. Less is often more.Medical evidence really isn't needed. If you have any evidence at all (even if you've sent it before) then send it, if it's relevant. They very rarely contact anyone. If they do then it will be your GP and they will be sent the UC113 form.Have you even started to make some notes? If you haven't then you should do that, at least you will then make a start.0

I have made some notes which led me to ask my question as my "examples" list is getting stupidly long.
I only moved to my GP a few years ago, just before the last assessment and when they wrote to the GP all the GP wrote was we haven't seen the patient with this condition ourselves but it's on his notes. And that was all they said0 -
Just a couple of real world incidents is fine. Don’t forget to include the detailed information.Yes a GP isn’t the best person to contact in my opinion.0

Right have decided next week going to make a start on this form, need to get it back by early July.
The only letter I have that mentions anything about what I am talking about is from 2014!! And from my doctor, while I fully understand about giving examples etc is it worth adding this in as well or is it so old it would be dismissed anyway?0 -
I always panic, I hate these things and I know the same questions get asked and the same answers given but its the reassurance I am doing everything I can I think.
I hate the way it works, its stress and anxiety filling the form in, the more stress and anxiety if you need an interview. This time I am trying to send absolutely everything I can while echoing the words of Poppy, less more. A fine juggling act I can tell you!0 -

Yes did the pip form, had the assessment earlier this year, was awarded pip a couple of weeks after assessment0

I’ve also applied for pip for the first time so I’ll be getting 2 forms next week . Feeling overwhelmed but I’ve asked for some help filling in forms . I’ll be sending them both as late as I can to give me all the time I need .Good luck with your form.0 -

logged in and it’s just a message from someone thanking me for giving them my GP details. 🤔I wasn’t so lucky, I’m not sure they contacted my GP , I was there today and DR mentioned nothing or if they did shes being really sneaky .0 -
Right so I get this straight. When completing the form, what do I write on the form for questions that have been answered on a separate piece of paper. So for example my reply is too big for the box on the page and I have written my reply on a separate piece of paper.
Do I then write in the answer box on the form, please see attached piece of paper or something?0 -

Hopefully one of the last few questions now!
So I have my doctors summary. It was the best I could get. So its like a summary record, just outlines what happened when I saw the doctor, any referrals made etc.
Am I ok just picking out the relevant pages and highlighting the parts that are relevant? Otherwise it will just be a wall of text

You can highlight the relevant parts but you will need to send all the pages. If you don’t they may wonder why you missed pages out.0

Ah right, its over 70 pages long
It's just the bit that says stuff like "say patient today, we spoke about xx and have decided to refer him them to xx"

That’s a huge amount of evidence. Is there anything in there that’s relevant that will help your claim? If not then ask yourself if it’s worth sending it.0

Its just the new stuff after my last assessment(which was 2018). It's new diagnosis, referrals etc. Most of it is just fluff like had blood test, here are results, came in to see about skin condition etc. Its only a few bits on a few pages which are relevant to what I am putting on the form.
Which is why I was trying to see if I could just send those pages rather than the whole lot0
