    Just put the evidence in the envelope with the form. You do not need to attach them to anything because they will remove the staples anyway.

    Can't believe this has gone on 6 pages. Just wanted to say thank you for all advice I have been given.

    I have finally finished the physical issues part of the UC50, now the hardest part to do. 

    I did receive a reminder letter today saying I need to return the form urgently. It needs to be with them by the 6th July.

    If I send it special delivery on Monday 3rd will that be enough time?

    They are usually returned straight to the Health assessment advisory Service rather than to DWP first. I don't know if that will be enough of time. If you get proof of posting then at least you have proof that you sent it.

    Just had another read of the letter it says it needs to be back with the health assessment people by the 6th so hopefully should be ok.

    Can I just check something about the address, the original letter said return in the envelope provided which had the address : 

    Centre for Health and Disability Assessments
    Bristol BSC
    Mail Handling Site A
    Wolverhampton
    WV98 2EL

    But the letter I received today said post to : 

    Bristol Centre for Health and Disability Assessments
    123 Winterstoke Road
    Bristol
    BS3 2NS


    So I assume ok to post to the last address ?

    The top address is a mail handling site. The bottom is the one straight to the health assessment providers. I’d use the bottom one.

    Just three questions to go! Will do them tomorrow and get the form sent off either tomorrow or Saturday, if it goes Saturday special delivery they will get it Monday.

    You're welcome. I must admit, that i also find the ESA50 form much more stressful than the PIP forms. Please do come back and let me know the outcome.

    Finally all done. Printed off and will be sent Special Delivery in the morning so will get their Monday. Phew. No idea of what the outcome will be so can only hope for the best.

    .. i also find the ESA50 form much more stressful than the PIP forms. ..
    I think that is because the PIP form makes some sense whereas the ESA/UC forms are hard because the questions seems daft. What job has ever required the repetitive moving of empty cardboard boxes at waist height and lifting small cartoons of milk.

    Well done for managing to finish the form, it is a difficult process for sure, now try and relax a bit you've done all you can until they let you know the outcome- I know that is easier said that done though.
