You can use extra sheets of paper to add additional people. They very rarely contact anyone anyway. If they do then they usually send your GP the UC113 form.

I did think that might be the case. I struggle so much with these forms, mainly in getting across the issues I have, same with the pip form, I really struggled to convey the issues. Due to some mental health issues I rush the form because I "have" to get it back as soon as possible, even though you get a month or whatever it is to do it, I try doing it there and then.0

I always find the WCA form to be worse than the PIP one. Usually i will give myself a timescale to complete it so i can do a little each day, without having to rush things. I always include a couple of real world incidents of exactly what happened the last time i attempted each descriptor that applies to me.For me i've always been in the Support Group (ESA) through mobilising descriptor.0

Good advice, going to try REALLY hard to do maybe a section a day or something as I really do want to get the issues across. I always end up in the LCW group which actually makes me feel a lot worse0

Just wanted to ask something about the comments in my journal. I am wondering if everyones is the same or .....
Mine says re referral ,is that just the same as everyone elses? :We have now taken the next steps to re refer you; this will be reviewed by the assessment team. You will receive a new questionnaire and then if needed, a further work capability assessment will take place. Please see the attached letter for more information.

Yup that seems correct to me, with the attached letter known as UCD35 to confirm the referral has been made.

Getting evidence is so hard, again keeping everything in one thread. I had the same issue with my pip application.
I wanted the councillor that I saw to be able to provide a backup to what I was writing down and the centre said they can't supply anything, just the notes from the sessions which was pointless as notes are only a summary and even then missed out or it wasn't included some of the things we spoke about and even if it was there it was like one line or something. I told them what I needed it for and nothing. How are you supposed to get evidence if they won't give it to you?0 -
You don't need medical evidence. Your anecdotal evidence is really all that's needed. A couple of real world examples of exactly what happened the last time you attempted each activity that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.My last ESA review was a paper based assessment based mostly on anecdotal evidence.0

Again sorry to ask this but really trying to keep this in one thread so I do apologise but will create a new thread if necessary. This question is really just out of curiosity after look at the form and the online sites showing LCW points etc.
So am I correct in thinking that if you take the "It varies" box on any question thats 0 points?0 -

Ah right its just that all the sites I looked at which show points and descriptors, none of them have anything for the "It varies" option so assumed it meant no points. Thanks for your advice.0

Also remember that if you can perform a task but you're unable to repeat it within a reasonable timescale then you're classed as being unable to complete that task.Think very carefully before you tick "it varies"0

Thank you Poppy, I remember doing the same form (but for ESA) years ago and think I ticked it varies on a few things that really I should have ticked I can't do that but back then had no support or help with filling the form in or anything so just muddled through. So thankful for this forum0

I think for me the main problem is articulating my responses on the form into something that doesn't sound like its a story from Jackanory (for those old enough to remember). ie I went to xx then I did this xx but this happened and xx
I was trying to see if there was any real world examples of how people wrote their responses, any question doesn't matter which to see how they formatted it and explained their issues but couldn't find any.0 -

It's hard to give template examples as each person's condition impacts them so uniquely. I vaguely remember Jackanorybut even if it sounds like a story , that's fine. Be formulaic in illustrating what the activity is, how your condition impacts your ability to complete it, if anyone helps, what would happen without their help, etc. Even if things are a given to you, having lived with a condition I know things do become autopilot! - spell it out in plain English so no assumptions can be made to fill in the blanks.
