In my area at least...
I received a Hospital appointment letter this morning
Posted 2 weeks ago for an appointment last Monday with orthotics!!
Luckily they had emailed me too but this was unusual.
Usually if a patient doesn't show up at an appointment they get black listed

They are having problems with recruiting and retaining postmen -

They are having problems with recruiting and retaining postmenhi @Wibblesmanchester has got exactly the same problem.i get letters sent really late like you, but technicaly minded people in manchester seem to be quite 'lucky' in some respects-we have got a service called 'my MFT' and its a service that gets linked up to all your GP,doctors,specialists etc,and it gives you all the letters long before you get them in the post. it also says if youve read a message or letter from a doctor- i think its to tell them your aware of the apointment but i have brain injury from my epilepsy and dont ever remember apointments from this unless staff are looking at it with me.it also gives all your diagnoses on the NHS, and you can write any aditional needs you might have in such as needing a bigger medical room for a wheelchair or a quiet room to wait in while visiting busy apointment rooms.i dont know if other NHS areas have this server software,and its not for everyone - like my parents for example both refused to get it as they are not good with computers.
