Coming off ESA and leaving home

I currently live with family and do permitted work at the weekend. I receive PIP(Daily Living Component: Standard and Mobility Component: Low) and ESA(Support Group).
I'm hoping to move out of my familys home soon but not sure what i'd have to do in regards to benefits with that? I've heard that you come off ESA and move onto Universal Credit as that includes housing benefit? So then i'd be on PIP and Universal Credit(and all involved with that like housing benefit. Is there anything else?)?
I would also like to up my hours working so would no longer be doing under 16 hours permitted work, meaning i'd no longer get ESA. But then if i'm working more(say 25 hours, am I still eligible for housing benefit?)
I'm just a bit confused since theyre both obviously two seperate changes so I dont know how it'd work out with my benefits all changing.
Realistically I would still work part time though due to needing to keep an eye on my health so I wont ever exactly be raking it in.
Could someone let me know what my best way to go about this is?
