Hi Poppy,
Form arrived in husbands name(some appointee) and filling it out due to me being overly on top of things and filling out a draft print out copy.Once complete does my son sign it or my husband?

As your husband is the appointee then he needs to sign the form. Same applies to PIP forms.

The part that says give permission for dwp to keep this info , does my husband allow this if our son says no?

As we went through form with him so he knows what it’s about etc as it is about and for him obviously

That question is asking if you want him GP to be informed of the outcome of the WCA.
Make sure you give as much relevant information as possible, without telling his life story. You should also include a couple of real world examples of exactly what happened the last time he attempted each descriptor that applies.

That's asking permission for them to contact his GP or other professionals for additional evidence, if needed. Though they rarely do this and if they do his GP maybe sent the UC113 form, which is a tick box form, with some others questions asking about diagnosis and other heallth conditions.

Is there any reason why you're thinking not to allow them to contact anyone? It's not a bad thing that they may contact them.
If you do tick no and change your mind you'll need to contact the Health Assessment Advisory Service and not put a message onto your journal.

On the form it says if you change your mind you can contact UC and it gives a number or by going to your online journal and report a change.
We have ticked yes anyway even tho son said no. But once we explained he is ok.

We wanted our son’s agreement with all the ticking etc as it’s for him but once we explained he felt ok about it.0 -
Also changing back to mine & husbands claim , they have now started to pay our landlord instead of to us for us to pass on to landlord which is one less thing we need to do.

To post the UC50 do I ask Post office for receipt of free postage? As I don’t want to risk it if they say they didn’t receive it , thank you

It's a specific thing called Proof of Postage. The Post Office will know what you mean. 🙂
